Forms needed before the school year begins:

  • Child Enrollment and Health Information

  • Child Medical Statement for Child Care

    The State of Ohio has updated the forms we need on file for each student. The state no longer requires a medical statement unless your child has a specific chronic health condition that may require the program to perform a medical procedure or administer medication.  The only medical information we need is immunization records or an immunization exemption form.

  • Child Medical/Physical Care Plan

    Only needed if your child has allergies
